Injuries Occurring on Campus
If you have sustained an injury or illness on campus, the Lafayette College Sports Medicine team is here to help. During team activities, student-athletes can seek triage and immediate care with the designated licensed athletic trainer on-site. We understand that injuries and illness can present insidiously or away from the facility. Please contact a member of our staff right away. This link provides contact information to our staff.

Going Off Campus for Care
Lafayette College Sports Medicine will never deny your right to seek a second opinion. Information on insurance claims and billing for both you and the provider can be found on our Insurance Forms page.
Returning to Campus After Injury
If you have sustained an injury away from campus and have been treated by a healthcare professional, please print the Release Form, complete, and submit to your treating healthcare provider. Records can be faxed to Lafayette College Sports Medicine at 610-330-5811.
Records Release Form
From your treating healthcare professional, we request the following items:
- All office notes
- All imaging reports (ex: MRI Report, Radiograph Report)
- Operative Report (If and when a surgery occurs, the physician will dictate details of the procedure to be transcribed into this report)
- Rehabilitation Protocol (This applies for time loss injuries or surgical patients)
- Prescription for "Athletic Training Services" from your treating physician (This allows us to continue your rehabilitation on campus)
- All physical therapy notes (This applies if you began rehab prior to your return to campus)
- Clearance Note (If you have been fully cleared by your physician, we request a note stating you have been cleared for "full, unrestricted activity". If you have not been cleared, we request a note specifically outlining your limitations, and which specific activities you are allowed to perform in rehab, the weight room, or sport-specific activities.)
