First-year student-athletes will receive information via email from their respective coaches regarding the incoming medical process. Forms are to be completed well in advance to the intended arrival. Please click on the First Year Athlete To-Do List link below for instructions and links to complete the process.

The process includes two parts: a Bailey Health Center component including form completion with a physical at home and a Sports Medicine component that includes completion of forms. If you do not receive an email regarding your Patient Portal account, please contact us at sportsmed@lafayette.edu. Your Patient Portal is set up after your Banner account/college email address have been set up.
