Proof of Insurance
Health Insurance is required for all Lafayette Students. Students have the option to purchase insurance offered by the school (SHIP) or provide proof of the own primary health insurance. If you have your own insurance and do not want to purchase the SHIP plan, you must opt out of the SHIP plan prior to Aug. 1 of each academic year. Insurance Form
As a varsity student-athlete, this form must be completed annually and submitted to Lafayette College Sports Medicine. All fields must be completed and legible. Boxes answering the first two question at the top must be checked. Images of the cards fixated at the bottom must be legible. A signature is required. This form can be loaded on Lafayette.medicatconnect.com using your Lafayette login, or be submitted to your team athletic trainer.
 

***This form must be completed whether you have your own health insurance OR if you have purchased SHIP. If you have two health insurances, the form must be completed twice.***

SHIP Insurance
The Student Health Insurance Plan (SHIP) is serviced by University Health Plans and new for 2017-18 will be underwritten by Aetna Student Health. To access your insurance card, visit https://www.universityhealthplans.com/ . There you can select Lafayette College's page and accessing your card is located under the Student Health Plan column on the left of your screen.
