Instructions on this page are intended for incoming student-athletes age 17 during their arrival on campus. If an athlete turns 18 prior to their arrival on campus, please follow the First-Year Student-Athlete instructions found on the Forms page.

There are two components to your clearance for participation in varsity athletics at Lafayette College; the Bailey Health Center component and the Sports Medicine component. The step-by-step instructions below will walk you through the process.

1. Your Username and Password are the same as Banner Student Services and your Lafayette email. Your account information for set up should have already been sent to you. If not you have not done this, do this first.

2. Once you have logged in, you will now be at the Patient Portal home screen. You will need your current insurance card, parent/subscriber's social security number, and parent/subscriber's employment information to complete forms. You may also need to print some forms from Patient Portal.

3. Patient Portal houses requirements for Bailey Health Center.

4. Click on FORMS under the To-Do List heading in the lower left corner. On this site, you will see several forms to be filled out. Please read, and type your FULL NAME and L# to acknowledge forms are complete. You must use the drop down box when entering the date. Should you receive an error message, please try again and refresh the page. If this error continues to occur, try to log out and log back in. If you cannot proceed, contact Lafayette Sports Medicine via email at sportsmed@lafayette.edu.

Complete the following forms:
Bailey Health Center REQUIRED FORMS:

* Emergency Notification Form
* Privacy Acknowledgment Form

5. You will need a physical with your home physician. From the Patient Portal, Download and print the "Physical Examination Form" and "Immunization Record Form" for a physician to complete. Please inform your physician, the NCAA mandates a Sickle Cell Test for ALL varsity athletes. The entire forms must be completed, including the status of participation.

***Prior to your arrival, the Physical Examination Form and Immunization Record Form must be uploaded. You can upload these forms by clicking the UPLOAD tab at the Patient Portal home page and follow directions.***

6. The Insurance Form is a required form and is to be downloaded from Patient Portal. Complete the top portion of the form. Copy and paste a copy of the front and back of your current health insurance card to the bottom of the form where indicated, then sign. This form must be uploaded. You can upload these forms by clicking the UPLOAD tab at the Patient Portal home page. A copy of this completed form should also be emailed to your team's coaching staff, as well. Also, it must be submitted by July 15th or prior to your arrival, if your arrival is before July 15th. If you are purchasing the health insurance offered by the school, please check the pertaining boxes on the insurance form. You must fill out the subscriber information with the student as the subscriber. Please sign the form with the parent/guardian and then upload it. You will receive your cards via email the first week of school.

7. At the Patient Portal home page, click on the Insurance Tab. Please read this form carefully. If you have already entered your own health insurance information on the Insurance Form, ACTION IS STILL REQUIRED. You must opt out of the school's health insurance by following the directions on the sheet. If you do not opt out, you will be charged for the health insurance. If you are an international student, you must purchase the school's health insurance.

8. Sports Medicine requirements (forms for varsity athletes) must be submitted directly to the Sports Medicine staff. These forms are on the Patient Portal, however, since the student-athlete is a minor, the forms must be printed, signed by the parent/guardian, and submitted directly to your specific team athletic trainer (see staff directory for the athletic trainer's email).
