New Student-Athlete Forms

First-year student-athletes will receive information via email from their respective coaches regarding the incoming medical process. Forms are to be completed by July 15. Please click on the "First Year Athlete To-Do List" link below for instructions and links to complete the process. The process includes two parts; a Bailey Health Center component including form completion with a physical at home and a Sports Medicine component that includes completion of forms. If you do not receive an email by July 15 regarding your Patient Portal account, please contact us at sportsmed@lafayette.edu.

Returning Upperclassmen Student-Athlete Forms

Please open the PDF titled: "Year End Instructions" to complete this summer's forms in preparation for next year. This is to be completed by the time you leave for summer 2015.

Year End Instructions Get Acrobat Reader

All returning athletes are given a flow chart that explains the insurance process before they leave for the summer and first year students are mailed this document with the Sports Medicine forms. All athletes also receive the flow chart during their pre-participation screening in the fall. Lafayette College's secondary insurance policy may help pay some of the medical bills that are not covered by an athlete's primary insurance policy.

These completed forms can be mailed to: Matt Bayly, 132 Kirby Sports Center, Lafayette College, Easton, PA 18042. It is highly recommended and strongly encouraged that you complete these forms through Patient Portal before printing and mailing.
